Signature Membership Form

Filling out the form for a Signature Membership is easy. Pick the option that is right for you:

  1. Fill out a form at your local studio, this is currently the only option for Premier and Choice memberships, or
  2. Download the Signature membership form, fill it out, sign, and fax to 214.393.3598, or
  3. Scan and email it to forms_only@sunstoneyoga.com, or
  4. Digitally sign and email it to forms_only@sunstoneyoga.com. Instructions are below:

How to fill out your membership form online, digitally sign it and email it to us to save time and avoid wasting paper. It is fast and good for the environment!
  1. Fill out as much of the form as you can. If you need help call 214.764.2119.
  2. Download and open the form with Adobe Reader or Adobe Acrobat. When you click the link to download it will most likely open itself.
  3. Fill out the following fields:
    1. Student Name
    2. E-mail Address
    3. Location - this is the location you will practice the most and where your membership will be assigned.
    4. Debit Amount - This is preset to $109. You should only change this if there is a current special or if you are instructed to do so.
    5. Start-Date - this is typically the day after your introductory offer expires.
    6. Auto-Debit Cycle Day - this is typically the same day of the month as your Start-Date. If you select another day of the month, you will incur prorated charges.
    7. First Month's Dues - This is preset to $109. You should only change this if there is a current special or if you are instructed to do so.
    8. Last Month's Dues - This is preset to $109. You should only change this if there is a current special or if you are instructed to do so.
    9. Initiation Fee - This is preset to $80. If you have received a coupon that applies to your Initiation Fee adjust the $80 accordingly. For example if you received a $20 coupon change the Initiation Fee to $60. If you are uncertain leave it blank and we will correct it and send it back to you.
    10. Bank Account information for subsequent monthly drafts.
    11. Credit Card Information for initial sign up payment- 1st month plus last month plus start-up fee.
    12. Initial twice where indicated
    13. Enter today's date
    14. Authorized Signature - when you press this box you will follow the steps below for digitally signing the document.
    15. Under Office Use Only, you may enter the name of the teacher who discussed your package options with you.
    16. After completing the form please email it to forms_only@sunstoneyoga.com. Please allow one business day for complete processing. If you go to take a class before your Signature membership has been fully processed, please inform your instructor of your online registration so he or she may enter you into class.
