- Any missing information will be corrected and emailed back to you for clarification, so don't worry if you are not 100% certain about every field.
- Be sure to select the appropriate Auto-Debit form.
- $109 Auto–Debit for Individuals.
- $169 Auto-Debit Family for all immediate family members at the same address.
- $82 Auto-Debit Under 25 for age 24 and younger, and seniors age 65 and older.
- Fill in the fields highlighted in blue.
- Email address for your name.
- For Location enter the Studio you attend most often.
- Start-Up Fee- if you are still in your Introductory Offer, you only pay $60.
- If you have received a coupon that applies to your Start-up Fee enter it next to the $80. For example next to the $80 type '-$20 = $60. If you are uncertain leave it blank and we will correct it and send it back to you.
- Start-Date is typically the day after your 10 Days for $10 expires.
- Auto-Debit Cycle Day is typically the same day of the month as your start date.
- If you select another day of the month, you will incur prorated charges.
- Bank Account information for subsequent monthly drafts.
- Credit Card Information for initial sign up payment- 1st month plus start-up fee.
- Initial and Sign the secure boxes.
- Under Office Use Only, you may enter the name of the teacher who discussed your package options with you.
- After pressing 'Click to eSign' you will receive a confirmation email verifying your email address. We will not receive your form until your email address is verified by you. Please allow one business day for complete processing. If you go to take a class before your Auto-Debit has been fully processed, please inform your instructor of your online registration so he or she may enter you into class. Welcome to unlimited yoga!
